Tetrabenazine has two modes of action; it depletes pre-synaptic stores of monoamines and has a postsynaptic receptor blocking action. These effects on the monoamine dopamine signal transduction pathways contribute to its activity.
The culmination of this activity is to modulate the transmission of electrical signals in the brain involved the control of movement. When given to patients suffering from involuntary movement disorders tetrabenazine's activity has been proven to substantially reduce involuntary movements.
Tetrabenazine is mainly used to control movement disorders associated with diseases of the brain and nervous system, such as Huntington's Disease. This is a hereditary and degenerative brain disease which is characterised by personality changes, mood swings, slurred speech, involuntary movements and an intoxicated appearance. The onset of the disease usually occurs in mid-life, when cells in the caudate nucleus of the brain begin to die, causing a deterioration of intellectual ability, balance, speech and emotional control. Huntington's Disease is caused by a single dominant gene. Every child of an affected parent has a 50% chance of inheriting the gene.
Tetrabenazine can also be used to control movement disorders caused by antipsychotic medicines. Older antipsychotic medicines such as chlorpromazine, haloperidol and flupentixol can cause side effects involving involuntary movement of the tongue, face, mouth and jaw, which may sometimes be accompanied by involuntary movements of the arms and legs (tardive dyskinesia). If this is disabling or socially embarrassing, and persists despite stopping or reducing the dose of the antipsychotic, the symptoms can be treated using tetrabenazine. Tetrabenazine can also be used if it is not possible to stop or change the antipsychotic.
